This page outlines Charmony Foundation’s community-based service delivery model and measurable program impact across the Inland Empire

OUR CORE PROGRAM AREAS

We provide hands-on education to help youth and families understand:
- Healthy eating habits
- Nutrition and food systems
- Chronic disease prevention (including diabetes awareness)
- Practical cooking and food skills

Our CHWs help families connect to essential resources:
- Care Coordination
- Health System Navigation
- Preventive Care Linkage
- Medi-Cal aligned Services
- CalFresh food assistance
- Medi-Cal enrollment support
- Housing navigation referrals
- Preventive health services
- Local community resources

Charmony uses existing community and school gardens to deliver experiential learning.
We do not require participants to travel far or rely on a single facility. Instead, we:
- Partner with existing gardens
- Conduct seasonal learning sessions
- Teach food systems, planting, and nutrition together

We support families experiencing:
- Food insecurity
- Chronic disease risk (including diabetes)
- Barriers to healthcare access
- Limited access to health education
Our goal is to strengthen long-term stability through prevention and early support.
