The Charmony Foundation’s Community Health Worker (CHW) Program is dedicated to improving access to healthcare and promoting well-being in underserved communities. By empowering individuals with the tools and training needed to address local health challenges, we create sustainable, community-driven solutions to improve overall health outcomes.
Our Mission To bridge the gap between healthcare systems and the communities they serve by equipping community members with the skills and resources to act as health advocates, educators, and facilitators.
What We Do
- Training Local Health Workers: We provide comprehensive training programs to equip
- CHWs are equipped with knowledge in key areas such as Chronic disease prevention, maternal and child health, mental health, and Parkinson Disease awareness
- Health Education: CHWs deliver culturally relevant health education to promote awareness and encourage proactive health behaviors in their communities.
- Healthcare Navigation: CHWs assist individuals and families in accessing healthcare services, understanding treatment plans, and navigating health systems.
- Community Outreach and Advocacy:At the Charmony Foundation, we are committed to building meaningful relationships and driving lasting change in the communities we serve. - Healthcare Navigation: Through home visits, community workshops, and collaboration with local organizations, CHWs identify community needs, advocate for resources, and address health disparities.
- Support Services: CHWs provide emotional support, help manage ongoing health conditions and connect community members to vital resources such as food banks, housing assistance, and financial aid programs.
- Mental health initiatives: Depression screening, suicide prevention programs, access to mental health services. CHWs educate community members on mental health conditions, reducing stigma and promoting understanding of available services and treatments.
- School-based health programs: Nutrition education, physical education, mental health awareness campaigns
- Maternal and child health: Early childhood development programs, breastfeeding promotion, prenatal nutrition education
- Chronic disease management: Diabetes education, hypertension control, heart disease prevention and management programs
