At Charmony Foundation (CF), we believe in the power of community, health, and harmony. Through our dedicated Community Health Workers (CHWs) and Community Outreach Programs, we work to ensure that everyone has access to essential health services, food security, and sustainable well-being.

(CF) is rooted in the mission to enhance the lives of individuals and communities within California and beyond. With a specific focus on underserved groups such as the geriatric population, veterans, women, and low-income communities, CF delivers impactful programs and services that address essential needs in community health, population health, food security, and global health.

Our commitment lies in creating equitable access to healthcare, housing, and essential social services, while also fostering awareness and education on chronic disease management. By sponsoring community organizations and events, offering mobile and office health screenings, and developing transportation initiatives, CF strives to fill critical gaps in traditional healthcare and social support systems.
